I do not write reviews lightly, but the pattern of conduct at this skilled nursing facility compels me to warn others. For over a year, I have documented repeated failures in providing even the most basic standard of care to my loved one. Despite multiple written notices and ongoing follow-up, the facility has failed to correct issues that directly affect her health, hygiene, and dignity. Among the most serious concerns: I have personally delivered specialized incontinence products that are clearly labeled, medically appropriate, and significantly superior to standard diapers. These were provided multiple times to ensure proper use. Nevertheless, staff repeatedly fail to use them. This is not a supply issue—it is a failure to follow care instructions and maintain basic hygiene standards. The result is unnecessary discomfort and indignity for the patient. Equally troubling, the facility has failed for over a year to provide routine dental and vision care. These are not optional services; they are fundamental components of maintaining a patient’s overall health. The prolonged absence of such care raises serious concerns about whether minimum standards are being met. Despite at least three formal written notices and repeated reminders, there has been no meaningful corrective action. Promises are made, concerns are acknowledged, and yet the same issues continue. This points to a systemic failure, not an isolated oversight. At the same time, the facility appears to have no difficulty ensuring that billing and payment continue uninterrupted. The contrast between their attention to payment and their disregard for basic care raises serious questions about priorities and accountability. Based on my experience, this conduct raises concerns about neglect and potential noncompliance with applicable care standards and patient rights requirements. Families should not have to fight this hard to secure fundamental, dignified care for their loved ones. If you are considering this facility, proceed with extreme caution. Ask detailed questions, document everything, and do not assume that reported issues will be addressed. In my experience, they may not be. I sincerely hope this review prompts meaningful change. Until then, families deserve to know what they may be facing.

Delta View Post Acute Rehabilitaion 1210 A Street Antioch, CA 94509 I received Excellent Care in Delta View Rehab facility after suffering a Broken and Replaced Hip. The facility itself was nothing to take note of as it had several 2 person bedrooms and several 3 person bedrooms. As with any Rehab facility, they had all of the equipment needed to assist in one’s rehab, plus extra requests from me. Sara, my case worker, and the staff of Delta View made my entire medical emergency a Very Easy process and left me with Great Respect for them. The Wound and Care Staff (Rosalie) was great in daily monitoring of my surgical wound, notifying the Kaiser doctors, cleaning, and replacing my bandages prior to Kaiser requesting that they be changed. Due to their daily inspections and treatments of my wound, she was able to clear up any possible infections while I was there and I have not had any problems with my wound whatsoever. Drug provider (Priscilla) was always on time for my various medications, and she took the time to explain each drug she was giving me. If I did not need a certain drug that day or time, I would just let her know and she would help to get me regulated (bowel, pain, and normal prescription drugs). She would also come find me when it was my time to take various medications, no matter if I was in different parts of the facility or just passing by her in the hallways. She apparently knew all of the patients and their medication schedules, as she always gave me my medications on time (in the hallway, in the room, or in the various treatment areas). The Nursing and Medical Staff (Norma, Consuelo, Eric, and several others) were always looking out for everyone’s best interests and were not very long in responding to our call buttons. Constantly monitoring my Blood Pressure, Temperature, and Pain levels day and night. Due to the nature of about 100 people all in Rehab together, it was not easy to get sleep at night, but I appreciated the Nursing and Medical staff checking up on me at all hours in an effort to catch anything that might come up like Infections, Medication reactions, Pain levels, Blood Thinners, etc. The Physical Therapists (Darius, Nita, Indy, Lambert, etc) monitored patient’s schedules, changed their schedules to accommodate the patients, found patients in various parts of the rehab facility, and provided workouts and routines to fit each patient’s needs. They gave me various exercises, equipment, and ideas in order for me to workout independently at my speed, and around the various things happening in the rehab facility. The Cleaning crews (Adriana and the 4 Marias) were also very good, and always cleaning the rooms, floors, etc. Everything was clean and smelling fresh, and they too would assist me with various tasks that I requested of them. To finish off, I would like to Thank the Chef (George) who came and introduced himself the first night I arrived. All through my stay, he would pop his head in and say ‘Hi’ and asked if he could get me something. He served up Very Good meals with a Variety of food groups that were different for each meal (breakfast, lunch, & dinner). Meals ranged from Crusted Fish with Rice, Chicken with gravy, Shrimps with Pasta, etc. They even had snacks at night. Keep in mind, I was only at Delta View 10 days….. Kahn & Veronica Alley

I spent time at Delta Vista Post Acute in Antioch recently, and I found the staff, nurses, rooms, and especially the Physical Therapists to be excellent. The nurses and other staff were very attentive to issues such as cleanliness and safety issues, and they encouraged people to participate in activities such as group physical therapy, movies, games, etc. Delta Vista has an excellent cook, and their food was very good and generous. (I especially miss their chicken salad sandwiches.) My stay there was a very good experience.

My mother was admitted to this facility following a femur fracture, and the level of care she received was exceptional. This rehabilitation center exceeded our expectations and does not reflect the outdated perception often associated with rehab hospitals. The staff consistently demonstrated professionalism, kindness, and encouragement, creating a supportive and compassionate environment throughout her recovery. In particular, her CNA, Satnam Kaur, provided outstanding care. Ms. Kaur was attentive, thoughtful, and consistently went above and beyond to ensure my mother’s comfort and well-being. This facility is fortunate to have such a dedicated and compassionate professional on its team, and our family is deeply grateful for the care provided.

Satnam kaur is a very best healthcare person I have ever met. She is just wonderful. She’s very attentive very caring very knowledgeable. Thank you so much. I will never be able to pay her enough. I’ve had wonderful care here from everyone, but she has been exceptional. Thank you. The building has been very nice and I’ve been very fortunate to have an outside bed where I could look out into the patio to the beautiful flowers. I want to take this hospital and the people in it and particularly said but sit down for wonderful care. I would recommend this place to anyone. Thank you Satnam

I am CURRENTLY a patient here for Rehab after a fall resulting in fractured ribs & vertebrae. The facility is clean!! The Staff is AMAZING! You never get the sense that they’re feeling put out when they answer your call light – for the BAZILLIONTH TIME. Food is beyond good! One meal included a slice of Apple Pie – WITH A GARNISH ON IT! I’m sure I’m gaining weight while I’m here. PT – great. Heading there right now!! Sasha is one of my favorite people here. The next time I fall & fracture ribs & vertebrae, I’m definitely coming back here. Just sayin’

⭐️⭐️⭐️⭐️ ⭐️ My mom has always been hesitant about rehab centers because she often feels alone in these kinds of places. But the staff here have truly changed that for her. They’ve been so caring, patient, and supportive ,especially Satnam Kaur (Sasha) and Dumeba who is new but is super eager to learn and is just so kind. They have gone above and beyond to make her feel like part of a family. Every day she’s there, she seems to get a little stronger, and I can see how much heart this team puts into their work. Thank you for opening your hearts, for your patience, and for making this difficult transition so much easier for my mom and our family. Also I have just named a few but truly they all work together and make is all just work!

I was transferred from Kaiser to this facility as other facilities were full. I had a bladder infection and needed daily iv antibiotics infusion. My medications were not available for three days: pain med, antocinstipation med and ibs med. I was lead on that coming on the way for days until finally the nurse told me that my meds were not available at their pharmacy in Sacramento and other two were too expensive to supply. I was asked for family to bring from home. Eventually Kaiser doctor got the meds delivered through Kaiser pharmacy which is a stone throw away. Have no pain medication made me suffer in level 8-9 pain. Not acceptable ! No one screened me for assessment and I was treated like a person who was not able to physically move. On the first day, two happy ladies came early in the morning to weight me. They brought a hoisting pick with a harness. When they lifted me up with the hoist my bladder and tubing were at the bottom getting crushed. I started screaming but they ignored it. They didn’t understand what I was telling them. What made it worse is a physical therapist came to move me to a wheelchair for workout room. My bottom was burning in pain and sitting down straight on it was not an option. I tried getting up but I was muscled down yes physically pushed into the sitting position by the PT. I am 70, have degrees, I am fully mobile but pt was treating me like I was a child refusing to cooperate. Looking back this was elder abuse! After many objections I still got shoved in and taken to the exercise room. There I was able to find a more reclining back position to get weight of my private parts. It still hurt but I pushed through it. Either that same day or the next day, two pt people came and I told them that I can walk but can’t sit. We went on a walk but when I was brought back to my room blood streaks were noticed in the catheter line. I contacted my urologist at Kaiser who messaged me to advocate for myself and that catheter and lines need to be taped up so not to allow movement inside. I checked the tape and it was not secure anymore as it came off and my walking did more damage inside. I have to challenge the description of skilled facility as I found it to be not. I was also on nebulizer treatment which I only got one the day before I left. Medication was always late as much as two hours. I was at the end of the nurses round I was told. One time a young nurse came in yelling at me that she’s busy but coming. This was very unprofessional. Once the nurse realized my physical needs my treatment vastly improved. Where was the assessment? One time I was stuck in a chair during deep cleaning for two hours or longer. I noticed my feet getting puffy so I dismounted from the chair and with a walker rested on my bed. That’s when everyone realized that I am able to move around with a walker. My every early morning diaper finger check for poo stopped too. I was constipated because both anticonstpation meds were too expensive. Did they substitute with anything else otc? Not! I had to use suppository on day 5. There are wonderful helpful people who work there but don’t have enough training to be “skilled” My GERD food had lemonade for days even when indicated. My neighbor was Chinese. She had language difficulties and I often heard her yelled at. I interviewed by telling the teller that she’s not deaf. She was crying often and used help to express her need. She was dismissed as an adult and treated like a child with force especially when taken out of bed. I would not recommend this facility to anyone. I got home and shortly after had to go to ER with blood in my bladder. These people should never be in charge of bladder infections and pain and catheter. Not skilled!

My grandmother recently stayed here for short-term recovery, and I am beyond grateful for the incredible care she received. I’ll admit, when my family and I toured the facility a year ago, we weren’t sure it would be the right fit. But after her most recent health situation, we decided to give it a try — and we’re so glad we did. From the moment she arrived, every single staff member was warm, welcoming, and attentive. You could truly feel how much they cared for the residents. I believe the compassion and dedication of the team played a huge role in her speedy recovery. Thank you to everyone who supported her healing journey with kindness and tenderness. I been to a few skilled Nursing facilities and so far Delta View Post Acute is the best I’ve been to. I have built a good relationship with the staff and the service is great and everyone is nice but the service I got from CNA “Satnam Kaur” is superb. She is super sweet and has a great heart. She makes sure your comfortable and she treats you like your her family. She is attentive not only to her patients but to all the patients. She walks up and down the halls when she not busy just to help anyone or just to poke her head in and check on you. She is always willing to help anyone no matter what the situation may be. She knows her stuff. Great teacher when students are here. She is awesome and a super nurse. It’s so messed up that she has to clean after CNA’s because they leave the trash full, they leave their patients a mess and she has to clean the patients cuz other CNA’s didnt complete thier jobs like they should. Delta View should never let her go. Please give her a raise and make her a manager. She is really true treasure and I hate when I see other CNA”s take advantage of her. RN Leandro and Michelae are the best nurses I’ve had. They make sure your needs are met. They both are very knowledgeable and helpful. Both are super nice. LVN Jocelyn is so sweet and loud. She loves to laugh and will keep a smile on your face. I love her laugh. She brings great positive vibes wherever she goes. George who is the chef is a great guy. He always accommodates to your food preferences (Food is good). He also jokes around with me and keeps me on my toes. I met a lot of the staff and they are all nice. Elvia, Yoni, Kayla, Jenna, Catherine, Jennifer, Lucia, both Norma’s, Maria, Joy, Lala, Arianna, Rosalee, Leslie, Donna, Erica. Darius, Laura, Marsha, Mia, Lambert, Celine, Orly, Victoria, Natalie, Kristen, Candy, Patricia, and etc… They make your stay easier so you don’t have to stress. I highly recommend Delta View Post Acute. I have been here 3 times and because I love it that much. If I have to go to a skilled nursing facility again it will definitely be Delta View Post Acute. Everyone here is my family.

I’m writing this for my mom, Mary Franchetto who recently spent 4+ weeks at Delta View for an injury. The staff at Delta View is excellent. We would like to thank Satnam, for her great care and my first bath! I also want to thank Elvia for knowing how to turn my mom without it hurting! Also a big thank you to Ashley, Renee, Jennifer, Michelae, Leandro and all the other nurses and CNA’s that cared for my mom. She especially loved her physical and occupational therapists, Darius and Indi and of course the other therapist there too. The Kaiser liaisons, Jackie and Sarah Mae were just as wonderful and helpful. All in all is was a great experience at a really hard time for our family. They treated us with care. There are more names we wanted to list but time has gotten away from us and we can’t remember them all! We would definitely recommend Delta View to other families.

I have been a patient I am writing this message to let you know how I have been treated here. I have been to many of this type a facility this has been the best of all. The rooms are clean. And they treat everyone everyone with respect she’s here satnam Kaur perfect CNA she is always here to help in every way she can. She has given me a shower and washed my hair for me. She responds to your call quickly. I would hire her as a CNA if I had a facility like this,I hire her and give her a raise because she deserves it. Also my physical therapist Indy is great. There are so many great people here it is hard to mention all of them my nurse is michelae she is also great.

I just want thank everybody at delta view for everything from the CNAs to the PTs and OTs it was such a positive experience for me sorry if I didn’t remember all of your names but just know that I’m thankful and grateful to you all just want to shout out all the therapist indy erica Darius Donna you guys truly care and wouldn’t have made such big progress without you so thank you I highly recommend this facility to anyone because they show that they care for you and tend to your evey need so again thank you so much delta view
